1、女职工身份证复印件(正面注明手机号);

2、《生育服务手册》或《生育证》原件及复印件;上述证件在省外(非户籍地)办理的,需提供全国统一制式的《流动人口婚育证明》原件及复印件 ;

如果是在私立医院生育,能报销吗?

可以报销。但是只能报销定额医疗费和生育津贴,生育并发症的费用就不能报销了。
